Affordable Care Act (ACA) Form 1095-C Notice

Charlotte County Public Schools is providing this notice in accordance with federal law regarding the availability of Form 1095-C, Employer-Provided Health Insurance Offer and Coverage.

Form 1095-C contains information about the health coverage offered to you by Charlotte County Public Schools during the prior calendar year. Form 1095-C is informational only and is not required to be attached to or used when filing a tax return.

In accordance with Internal Revenue Service regulations, Charlotte County Public Schools will not automatically mail Form 1095-C to employees. However, you may request a copy at no cost.

Timing of Delivery

If you request a Form 1095-C, a copy will be provided to you within 30 days of the request, as required by Federal Law.
